The miniseries shouldn't be in the Specials. There's an entry for it here: https://www.themoviedb.org/tv/71365 And the episodes should be in the release order..

Should Razor and The Plan be included both as specials to the 4-season series and standalone movies, as they are now?

Razor is probably fine as it it. The special had a limited theatrical release prior to its TV premiere.
