Extra Details: As visible for example on this snippets of the November 27, 1997 TV schedule : 1 and 2, the second pilot movie (episode 2) was originally broadcasted under the "The Return of the Incredible Hulk" title.
It was only renamed later to "A Death in the Family".

When I watch the episode, it shows "DEATH IN THE FAMILY" at 1:34. What is considered the proper title? Is there a definitive source? A broadcast database, or just from memory?

The proper title is the one used for the original broadcast as it was advertised by the broadcaster at that time (links are in the first message).
