Nach einem Unfall kommt Harm Rabb in ein Krankenhaus, in dem zur gleichen Zeit unter strengen Sicherheitsvorkehrungen ein hochrangiger israelischer Politiker für eine Herztransplantation vorbereitet wird. Doch trotz der Bewachung dringen fünf islamistische Terroristen in die Klinik ein. Der Anführer Mustapha nimmt sofort Geiseln, während seine Komplizen die Notausgänge mit Sprengsätzen unpassierbar machen. Trotz seiner Verletzung beginnt Harm einen Feldzug gegen die Terroristen.

Hospitalized with a knee injury, Harm is forced to take action when the Hamas, a terrorist Islamic Resistance group, takes the hospital hostage.

An Israeli diplomat undergoing a heart transplant provides the Hamas terrorists with the perfect negotiating tool. They take over the operating room -- as well as the entire hospital -- and use the diplomat as leverage while demanding the release of one of their imprisoned leaders. Meanwhile, Mac is trapped in the hospital cafeteria trying to negotiate with two terrorists.

Mentre fa una corsa con Mac, Harm viene investito da un'automobile ma spingendola via salva la collega dalla stessa sorte. Mac lo accompagna in ospedale, l'incidente non ha avuto conseguenza gravi, e mentre aspetta la prognosi definitiva alcuni terroristi di Hamas la prendono in ostaggio insieme ad alcuni medici che stavano per operare un ministro israeliano. I terroristi minacciandolo di morte vogliono la liberazione di un prigioniero palestinese. Harm e Mac decidono separatamente di intervenire. Harm deve però, contemporaneamente, pensare ai terroristi e a una donna incinta. Fortunatamente lo aiuta un vecchio marinaio in pensione.

Le lieutenant Rabb est loin d'imaginer que son habituel footing matinal va se terminer à l'hôpital. Au même moment, Semadar Baruk, un émissaire du gouvernement d'Israël, subit une intervention dans ce même établissement. Soudain, un commando du Hamas investit les lieux...
