Why is his head so small? Did his original head get crushed in some sort of accident, and they had to take the replacement from a clone that didn't have time to mature fully? That's my theory.

Nah, Mikey's just thin and slender, like Alex or Chris.
Just look at when Mikey, Alex, Chris and some of the rest of them portray U.S. Senators. None of them are going to have a head as big as a real senator, but it's their characterizations which save the day if not the country.
