Hi! We always try to follow the original order. "You're My Home" is Episode 24: https://web.archive.org/web/20150516002914/https://abc.go.com/shows/greys-anatomy ("She's Leaving Home" is a two-hour episode).

The link is not outdated, its the archived version of the official website from when season 11 was originally airing on TV. "You're My Home" was originally broadcast as episode 24. There's nothing to change.
