Report radio buttons don't always work

Sometimes it's impossible to select a report type using the radio buttons. I found a few references to this in some report threads, but did not see an existing thread about the issue in this forum.
For example, when creating this report just now, I had to use the browser devtools to edit the form and manually add the "checked" attribute to the radio button for "Incorrect content" because clicking the radio buttons did nothing.
This has been a recurring problem on the site all week, for me. Disabling browser extensions doesn't fix it.

Wow yes! It seems like we're unable to report with seasons and episodes. Though, if you try, and then hit one you want to check, and then close, and hit report again, it IS checked.... so weird.
Hopefully, @Travisbell can fix it!
