TL;DR: it seems impossible to get off the blacklist.

It seems to be sporadic. e.g. I have shared images, links etc. before now when it comes to TMDB.
I will try again at some point in the future and see if I can get the same result with a different image.

Point of note on this one:
This is okay: https://image.tmdb.org/t/p/w533_and_h300_bestv2/bNohyqMPZaU9YDHmaQsyAFsJ2to.jpg
This is not: https://image.tmdb.org/t/p/original/bNohyqMPZaU9YDHmaQsyAFsJ2to.jpg
It isn't image.tmdb.org which Facebook has issue with. It is the original folder within that sub domain that it has issues with.

No. Facebook dislikes really large images during peak usage times. On occasions like that, if it has never seen the image before, it will fail to give you a preview. That said, it won't throw a warning.
I can only guess, that at some point, it encountered something it didn't like within the Original folder.
Or is it a header issue? e.g. I am guessing for a resize the headers are different to when you just farm out the original image? I am not sure what you are doing behind the scenes, but does original behave differently to the other pages? I will have a gander at the headers.
