I believe that the American version of this movie and the Italian version should be separate films here in TMDB. They have different titles, different writers, some of the acting roles are entirely different, a different story, scenes shot differently for each movie, different editors, different music.

It seems clear that these are essentially different movies and should be two separate titles/films here.
