The "Translated Title" field is for the localized title for that language and country upon release. "The Boy and the Heron" is the title used by GKIDS for the US so the English title is correct.

GKIDS do not pick the title, they're not the producers of the film, they're just distributing it in the US. The correct title of the film is How Do You Live? and the TMDB entry should reflect Miyazaki's choice in this, not GKIDS'. Please change this to the official translation of the movie's actual title.

The US (English) title should not matter since this a Japanese film.

The English translated title has nothing to do with the production. As indicated by @raze464 above, it should be the title used for the first release in an English speaking country. Consequently, the current English title is correct.
