Set in Naples at the end of the 19th century, I Guappi tells the story of a strange friendship between the powerful and charismatic Camorra boss, Don Gaetano (Fabio Testi), and one of the many scugnizzi who grew up on the streets, Nicola Bellizzi (Franco Nero). Nicola, thanks to Don Gaetano's protection, becomes first an honoured picciotto of the Camorra and then a lawyer. He tries to break away from his 'family', but falls back into a life of crime and violence.

Le jeune Nicola Bellizzi s'installe dans un quartier misérable de la fin du XIXème siècle, de Naples, qui est sous le contrôle de la Camorra. Nicola est issu du même milieu que Don Gaetano, le chef du quartier. Nicola sort d'un long séjour en prison où il a nourri l'ambition de devenir avocat. Sans argent, il est obligé de jouer aux dés pour s'en sortir, ce qui ne se fait pas sans l'autorisation de Don Gaetano. Les deux hommes s'affrontent mais Nicola le défend lorsque la police intervient pour arrêter le duel. Ils deviennent alors amis et Nicola rentre dans la Camorra en tant que second de Gaetano tout en continuant à étudier le soir. Pourtant les méthodes employées par l'organisation secrète le dégoûtent. Il essaie de rester le plus possible en marge du groupe, se vouant à la défense des pauvres, sans pour autant renier son amitié pour Don Gaetano...

Um die Jahrhundertwende in Neapel: Als ein junger Mann nach sechs Jahren Haft wegen revolutionärer Aktivitäten in sein Stadtviertel zurückkehrt, wird er Mitglied der Camorra. Dadurch kann er sich seinen Traum, Rechtsanwalt zu werden, erfüllen. Doch er gerät durch seinen Gerechtigkeitssinn immer wieder mit dem mit brutaler Gewalt herrschenden Camorra-Chef aneinander.

Napoli, ultimo decennio del XIX secolo. Nicola Bellizzi, dopo essere stato uno dei tanti scugnizzi senza famiglia cresciuti per strada, dopo essersi fatto un nome come "Coppola Rossa" ed essere finito in riformatorio, decide di dedicarsi con tutte le forze a studiare per diventare avvocato.

Vorrebbe mantenersi fuori dai guai, ma appena arrivato nel nuovo quartiere si scontra con il boss locale, Don Gaetano. Dopo un duello a colpi di rasoio, interrotto dall'arrivo della polizia giusto prima che scorra il sangue, Don Gaetano si convince che Nicola sia un uomo di valore e lo prende sotto la propria protezione, presentandolo al gran consiglio della camorra che lo affilia come picciotto onorato. Nicola, per poter perseguire il proprio ideale di giustizia, si ritrova così suo malgrado legato per la vita alla criminalità.
