How was that pulled off so swiftly without Casey seeing anything? Actually, I think she must have seen something because she squirms away for a second before turning on the patio light, but did Stu just run across and slit his guts open and then hide somewhere else, or did it happen another way?

Maybe she did see a dark figure running by, the camera just doesn't show us. But it IS more difficult, ofcourse, to see what's happening outside at night when the lights are on inside. I think Stu was probably hiding on one side of the patio, ran across and slit Steve's guts and then hid on the other side.

Movie logic. Same as getting us to believe that one person was responsible for the entire thing until it was revealed there were two killers at the end.

If you're talking about Roman. He was not involved in any of the murders in the first movie. As he said himself, Billy and Stu decided to make a movie of their own.
