Did Elvira really have cancer? There's a scene where Elvira, passing off herself as Virginia Goodman, says that Tomas had to take care of his wife Elvira, because she was sick.

She was. The first time in the movie we hear about it is actually much earlier, when Tomas drives Laura to his house in order to repair the car.
Of course, there's no guarantee that this detail wasn't invented by somebody along the line. After all, we the audience only hear about it in retrospect from Adrian, talking to Virginia, relating to her what Laura (supposedly) told him about meeting the parents the night of the accident. Any of those chain links could be fake or have invented details, in theory. It just doesn't make a lot of sense for one of them (Adrian or Laura) to invent the mother's cancer. What would be the point?
