Hughes was known to be obsessively/compulsively clean (amongst other mental health issues).

This probably just signifies a traumatic emotional event triggering a period of mania.

Scorsese's screenwriter John Logan coulda envisioned Hepburn for Howard as a "dirty birdie" like in Stephen King's novel.

And HH mightve seen himself as a phoenix, risen from the ashes.
