We know the Doctor had to kill Adam before 6 to live, but how was Adam meant to get out? Was he there solely for the Doctor to kill or was there a way out for him that I've missed?

He had a key with him in the water but went down the pipe when he woke up. This can be seen at the beginning but people usually misses that. In the end, when John is leaving the room he says there's a key in the bathtub and then the scene of the key disappearing replays.
He was able to saw his foot too, but he trusted the Doctor.
