At the cafe, if I'm not mistaken, one of the killers appears to be a middle-aged woman. How's that?
She might have lived in another town and the kids let her live for helping them?
She's just an older-looking kid? There's another one of them that looks like he's 35.

I too wondered this about the woman in the opening scene. She's the one that poisoned the coffee. At first I thought maybe it was a young teen waitress or something, but you're right, it was an old-ish woman. I think you're probably right in that the kids had an agreement with her to let her live, like with the mechanic.
