John J. Rambo pricestoval do amerického provinčného mestečka Hope hľadať priateľa z vojny, s ktorým bok po boku bojovali v špeciálnej jednotke vo Vietname. Prichádza však neskoro, pretože jeho priateľ medzičasom zomrel na následky chemickej zbrane známej pod menom „Agent Orange“, ktorej pôsobeniu boli vystavené aj americké jednotky bojujúce vo vietnamskej džungli. Keďže polícia považuje viacerými vyznamenaniami oceneného vietnamského veterána za tuláka, zatknú ho a posadia do väzenia. Miestny šerif Will Teasle netuší, že tým spustil smrtonosnú lavínu – pretože prebudil Rambove inštinkty! Tomu sa totiž v dych vyrážajúcom tempe podarí uniknúť z väzenia a na ukradnutej motorke si to zamieriť do neďalekých hôr. Keď tam dorazí, ocitne sa vo svojom živle: a tým je guerillská vojna! V tomto štádiu už ani Rambov bývalý nadriadený plukovník Samuel Trautman nedokáže oboch mužov zastaviť v ich boji na život a na smrť...
