約翰·蘭保（John Rambo）是一名從越南戰場歸來的美國陸軍綠扁帽退役隊員，回國後，他想尋找以前的戰友。結果卻發現戰友因為接觸美軍噴灑的落葉劑致癌而早已去世。對於沒有親人沒有家庭的蘭保來說，戰友的去世讓他十分寂寞和消沉。當他漫無目的地在公路上徘徊時，當地的警察前來盤問。他回答說自己正打算去市區找一家餐館，但是警長將他帶到城外，他因此反抗性的走向城內。於是，維護地區治安的警察便以此把蘭保帶回了警局。

回到警局後，警察們開始對他進行侮辱式的查問。蘭保所受到的屈辱讓他回憶起在越南戰場被北越軍殘酷拷打的場面，他因此失控並打倒了數名警察，之後搶走摩托車逃入了市郊的樹林中。小鎮警察展開追捕，卻慘遭痛擊─先是一名欲射殺他的警員自直升機上失足摔死，其他人也被蘭保利用叢林之利弄傷，直到此時，他們才知道這個「逃犯」的特種部隊身分，還曾因越戰中的表現優秀而得過榮譽勳章。

蘭保的襲警成為了新聞事件，而他在越南的前上司崔普曼上校也因此前來勸說警方罷手並嘗試勸降蘭保。然而意欲報仇的鎮警長不顧勸阻，並召集了國民兵支援，將蘭保趕入一處礦坑。但就在大家以為蘭保已被炸死在礦坑內時，早已逃脫的他手持M60通用機槍單槍匹馬回到小鎮，接連炸毀了加油站與野外用品店，並把警察局局長打得重傷。然而，當崔普曼上校現身勸他投降的時候，一直沉默無語的蘭保終於喊出了越戰老兵對於社會的不滿和痛苦。

一羣冷血的電單車飛車黨在公路上肆虐，警方亦束手無策。其後探員麥斯在一次追捕行動中導致飛車黨的成員在車禍中死去，其飛車黨夥伴誓言報復，令麥斯的妻兒慘死。麥斯決定不惜一切進行大報復⋯
