Den amerikanske specialagenten Leon S. Kennedy smiter in i ett litet östeuropeiskt land för att kontrollera ryktena om att bioorganiska vapen (BOW) används i krig. Precis efter infiltreringen beordrar den amerikanske regeringen honom att omedelbart lämna landet. Fast besluten att ta reda på sanningen ignorerar Leon befallningen och ger sig ut på slagfältet för att avsluta raden av tragedier orsakade av de bioorganiska vapnen.

Ett antal högt uppsatta fascister kidnappar några ungdomar och för dem till en avlägsen plats för att sedan förnedra, våldta och tortera dem. Berättelsen bygger på romanen "Les 120 journées de Sodome, ou l'École du libertinage" av markis de Sade från år 1785, men handlingen är förlagd till den s.k. Salòrepubliken, det av Nazityskland ockuperade Norditalien 1944–45.
