影片前三部讲述了近未来的美国，全国失业率仅为1%，犯罪率则创下史上最低纪录，暴力几乎消失无踪。美利坚合众国仿佛进入宛如天堂的美好黄金时代，而促成这完美表象的则是前所未闻的大扫荡计划。国家为了有效释放公民心底的压抑和不满情绪，特别规定每年有一天的晚上7点之后举行长达十二小时的全国性大扫荡，届时人们将走上街头，展开完全不用承担法律责任的杀戮。

电影聚焦于导致清除计划发生的原因，讲述了由于人口膨胀，社会问题加剧，一个名为New Founding Fathers of America的组织进行了一种社会学理论测试，即在某一孤立的社区进行为时一晚的发泄、攻击、侵略，促成社区人们之间的相互残杀。这一制度在全美推广，一时间马路街道都成为了杀戮的战场。故事的荒谬性吸引了大量观众，第一年的杀戮日就要降临，恶魔们都已经准备好了。

故事紧接《人类清除计划3》。从贩毒集团手中脱逃的阿德拉与丈夫胡安自墨西哥来到美国得克萨斯州生活，后者在塔克家族的牧场工作。胡安深受塔克一家的族长，凯莱布的赏识，但这也让凯莱布之子迪伦心生妒忌。但在一年一度的“人类清除计划”过后的早上，一支地下组织意图将杀戮继续进行下去，让全美陷入狂乱之中。包括迪伦妻子和妹妹在内的塔克一家也遭受波及，迫使众人联合起来对付暴徒，并设法逃入开放边境仅有六小时的墨西哥。
