Den 12-årige Ofelia rejser med sin gravide mor til skovene i det nordlige Spanien for at bo hos morens nye mand Vidal, en sadistisk kaptajn i Francos hær. Da Ofelia udforsker en stor stenlabyrint, viser den sig at være hjem for skovguden Pan. Pan overbeviser hende om, at hun er prinsesse i et hemmeligt undergrundsrige. Hvis Olfelia ønsker at vende tilbage til sit kongerige, så må hun udføre tre livsfarlige opgaver.
