The big difference is "mother!" is the official capitalization for the film. It was used in the opening credits, on the official website and social media accounts, by reviewers, etc. and the intentional lowercase was confirmed by the director.
Many posters have unusual capitalization for purely aesthetic reasons, but it's not the official movie capitalization. I don't think "a sister" should be used here. All sources I've consulted in both languages use standard capitalization for the film.
