So the baron wanted a pure bloodline, right? So he developed a "cure" for his sister's infertility, and then she eventually got pregnant. But she was killed when the peasants burned the castle down, and the baby was thrown into the magical eel waters.
What exactly was the baron's ultimate plan? To live forever, and impregnate his daughter, then impregate that daughter, and so forth?
If he wanted his daughter to reach womanhood, then why had he been giving her the "cure" for 200 years (knowing that it slowed down the aging process)? Why didn't he just let her age normally, and THEN give her the cure afterwards?

He wanted a fresh young daughter, hence the creepy fingering and sniffing. That went a bit farther than I thought necessary but probably not unrealistic for a bastard of the baron's ilk. I honestly didn't care about the plot, but focused on the excellent sound design, creepy atmosphere, awesome cinematography, and social tension throughout the film. Overall I thought it was pretty good for one of those original oddball flicks that will achieve cult status somewhere down the road.

It makes no sense ha! I'm still wondering why there were so many people using 'the cure' since it's true purpose was helping the baron reach his goal. Anyone got a clue?

Yeah, I didn't really follow the plot all that well and when the whole thing was said and done I wanted to watch the movie again just to see if anything I just saw even fit together. But it's a really good "atmosphere" movie and it gets very disturbing, at times probably too much so but it still worked for me.

It did have excellent atmosphere throughout and agree about the spam I remember seeing that guy on IMDb all over the place, but at least there we all had signature options so it was ok, but here, do we even have sig options on this site? I hope so if not typing that repeatedly on the end of each post is really a bit insane.

I'm guessing he'd cut & paste rather than type it all over again every time.
The movie did a great job with atmosphere and mood for sure but the hollow plot devices, shaky motives, nonsensical 'science' and the burden set upon me to suspend disbelief against all odds makes it one that I cannot recommend.
It is unfortunate that they did not have a more experienced or capable writer to consult before moving ahead. I think the right person may have been able to untangle this knot of a script.
All other elements were well done and I could feel the tension and sense of isolation grow throughout the film. They did so much right that it is a terrible shame that, ultimately, it went so wrong.

Tha baron's goal was to have a heir I guess, otherwise the movie went for too long and I didn't understand the skeletor face.

Not how I saw it.
The Doctor had invented a method, a “cure”, to distill a life essence from his unsuspecting patients, who would slowly become weaker and weaker (loosing teeth etc.) to the point where they would eventually end their lives as fodder to the eels living and thriving in the underground water reservoir of the Sanitarium. This water, in turn, was an integral part of his “cure”.
This “cure” was essentially slowing down ageing.
He had done so for the last 300 years at least, meaning he and his peps were older than 300 years. How much, I do not know.
Under the cover of a fancy pants sanitarium he would keep on getting fuel for his essence. Read; wealthy old people would keep on joining unsuspectingly.
He had once been burned severely (300 years ago) by angry mob-villagers and so had found a way since to make a humanoid looking mask for his face.
On top of this, he was obsessed with keeping his bloodline clean and so kept his sister captured so he could procreate with her. 300 years ago when the angry villagers had burned him, they also burned and killed his sister. Though, their unborn daughter survived her mother’s death partly because the fetus was dumped in the water reservoir (which had heeling properties that he later (or already did) would use in his “cure”). This baby is the girl we meet. In other words, she had been slowly aging over the last 300 years to become a teenager by the time we enter the story. And apparently this whole situation is unknown to her. Or she was kept in the dark. Since The Evil Doc is still obsessed with keeping his bloodline clean, he planned to procreate with her once she had had her first period.
Perhaps, having sex was a no no, as he was so adamant on keeping the blood line pure. Remember the couple masturbating. Why put this in? Well, perhaps because intercourse was against the mind of the boss.... only he was allowed, and only with his own kin.
So in short, his plan was to live forever and have sexy-time with his own kin. In other words, batshitcrazy.

HAL - It seemed that the wealthy old folks were happy to submit. I think the promise of life extension, even if it wasn't their own, and the idea of a 'pure' bloodline was the appeal. Although, like most of the movie, the motives aren't clearly defined. Really, the whole movie is like the barons plan. It seemed like a good idea in the beginning but turns out to be a shitshow.

I did not get this at all. Although now that you say it it does explain one or two odd moments that would otherwise not be explained so easily.
The daughter part was odd. Seemed a bit like a last attempt to put in some element in the story to make it more interesting.... if they instead would have just build a story about a welness spa that actually sucked their patients dry of life, would have been a better story... so let say our hero was an actual patient who started to see their scheme...

Now that you say it, I agree completely. The whole thing was misguided. I would have much preferred to watch the movie you are describing.
