Vietnam savaşından sonra bir grup Amerikan askerinin Vietnam’da esir olarak bırakıldığı haberleri basında yer almaya başlayınca eski ABD Ordusu askeri John Rambo eski komutanı Albay Sam Trautman tarafından hapishanede ziyaret edilir. Hükümet bu haberlerin doğrulanması için özgürlüğü karşılığında Rambo’nun yardımını istemektedir. Görevi kabul eden Rambo, yapılan plana göre düşmanla sıcak temasa girmeden sadece bilgiyi doğrulayıp ülkesine geri dönecektir. Ancak işler planlandığı gibi gitmeyip kendi ülkesi tarafından ihanete uğrayınca, Rambo tekrar geldiği bu cehennemden kendi olanakları ile kurtulmaya çalışacaktır.
