In the premiere episode, the Chief of Surgery ends up in a coma, and his fiancée and fellow surgeons fight to save his life.

Alex Reid treats a young boy with mysterious symptoms while dealing with the arrival of Charlie's ex-wife. Elsewhere in the hospital, Joel Goran and Maggie Lin try to help a patient who refuses life-saving treatment on religious grounds.

With Charlie's life hanging in the balance, Alex and Dawn go head-to-head over who gets to be his legal decision maker - but are forced to put personal matters aside when they scrub into a heart transplant surgery together. Charlie, meanwhile, is in no mood to talk to the heart donor - a deceased prisoner.

In the Season 1 finale, Alex confronts game-changing questions following a monumental turn of events. Elsewhere, Joel scrambles to save a patient suffering from a rapidly spreading flesh-eating disease.

Alex and Charlie are caught in the middle of a shooting on their way to their first shift back at Hope Zion. In the midst of the commotion, Alex treats a bullied teenager and deals with her own emotions about everything that’s happened in the past year. Meanwhile, Charlie is caught off guard when he realizes that he’s still able to see the spirits of deceased patients. As well, new Chief of Surgery Joel works alongside Charlie’s ex-wife Dawn to perform an intense surgery.

When Alex and Joel end up stranded in the middle of nowhere with a severely injured couple, they’re forced to use only what’s around them to stabilize the pair until paramedics arrive. Meanwhile, back at Hope Zion Hospital, Charlie must try to save the wounded leg of his former college roommate as Gavin attempts to help Shahir improve his bedside manner.

Alex risks a dangerous surgery on a seemingly inoperable tumor to save a young boy’s life. Meanwhile, Joel struggles with the challenges of being Chief of Surgery as Charlie struggles with the spirit of Alex’s dead brother Luke.

Alex is diagnosed with an irregular heartbeat but must care for the Great Randall; Charlie deals with the aftermath of the fight; one of the staff is attacked.

Alex was stabbed in the chest and Charlie sees her as ghost. Alex realizes she saw her father hang himself. Joel has issues with himself about Alex. Maggie end hers and Gavin's relationship. Alex end up in coma and remains ghost.

Meanwhile, Charlie and Joel square‐off over differing treatment options when two fused-together burn victims are brought into the ER, and Maggie finds herself in a whirlwind of emotions triggered from the fallout of her miscarriage.

Alex returns to work for the first time since her stabbing. She is introduced to her buddy colleague, as she returns. Joel ends up involved in a deadly situation at a local street clinic. Gavin shares a euphoric experience, with someone unlikely.
