Confused by the ending of this. When he puts the needle to his neck, and then we see him at the tunnel with the locket, what exactly are we meant to think?

they said people with dementia twist memories to their benefit, so you can't take literally everything you see through his eyes
it's basically Fight club, except here the young guy actually exist but did nothing bad
