You can find the same style in a short video called "End of Evangelion DVD Menus" here, at 0:38 min:

In this single image, they use the word "AND" - but in all the other graphics they use "&". You can also find this style in posters, for example this (distorted) one (left upper corner):

I did find a stream of the full movie and skipped through it -> the title card has the same style. But since there are 2 parts (part 1 = Death, part 2 = Rebirth), I didn't spot both title parts together on screen. So far. I can look for this again, but, honestly, is it worth it? Both forms are definitely in use, and it's a mere decision to choose one of them as original title and put the other in the alternative section. I don't think one of them is "more right" or "better" than the other one ... since it's a "translation" of the stylized graphic.

I suppose, it's just a bit unusual that there are two different variations of how they've officially titled it, normally you only have one style or the other.
