Een vrouwelijke boer gaat op bezoek bij haar zieke zus in Bogota. Daar sluit ze vriendschap met een Franse archeoloog die toezicht houdt op een bouwproject en ontmoet ze een jonge muzikant. 's Nachts wordt haar slaaprust op mysterieuze wijze verstoord door luide knallen.
Oom Boonmee lijdt acuut nierfalen en hij weet dat hij binnen 48 uur zal sterven. Hij laat zich door familie naar een onherbergzaam gebied brengen om te sterven, waar zij geconfronteerd worden met de geest van zijn gestorven echtgenote. Ook zijn verloren zoon komt tot hen uit de jungle, in de vorm van een aap. We volgen de laatste dagen van Boonmee tussen zijn familie en hun verhalen.
De film vertelt twee verhalen. Het eerste gaat over een vrouwelijke arts in een ziekenhuis op het Thaise platteland tijdens de tweede helft van de twintigste eeuw. In het tweede verhaal staat een mannelijke arts centraal. Hij werkt in een medisch centrum in het hedendaagse Bangkok. De dialogen in beide verhalen komen globaal overeen, maar de uitkomsten verschillen.

Gezellige avonden bij Tongs familie, nachten vol zang in de stad... Tot dit leventje plots overhoop gegooid wordt door een verdwijning. En één of ander wild beest koeien afslacht. Plaatselijke legendes beweren dat de mens op de één of andere manier kan omgevormd worden tot een ander wezen... En dan begint het verhaal van een soldaat die alleen diep de jungle intrekt, waar mythe vaak realiteit blijkt te zijn...
In een kleine stad in Thailand melden zevenentwintig soldaten zich met een vreemd geval van slaapziekte. Een verlaten basisschool wordt daarom omgebouwd tot slaapzaal. Jenjira Winder is een Thaise vrouw van middelbare leeftijd die getrouwd is met Frank, een gepensioneerde soldaat uit de VS. Ze meldt zich aan om de soldaten te verzorgen en ontwikkelt een speciale aandacht voor de jonge soldaat Itta, die geen familieleden op bezoek krijgt.
